First Care Clinic of Chattahoochee provides a wide variety of services in the Primary Care Scope of Practice. This practice has been serving our area for over 60 years. We offer management of Chronic illnesses like Hypertension, Diabetes, High Cholesterol, and many more. Plus provide Urgent care for minor lacerations, burns, and infections. We are currently accepting new patients, same day appointments, and walk-ins are always welcome.
Brian is a lifelong resident of Jackson and Calhoun County and is married with 3 children. He enjoys fishing, hunting, farming, and camping in his off time. We are currently accepting new patients to this practice. To make an appointment please call 850-793-1100 and our hours are Monday thru Thursday 8am to 5 pm and Friday 8 to noon.

First Care Clinic of Chattahoochee is proud to have Brian Ham, APRN-BC, Practicing Primary Care with a focus on Adults and Geriatrics in West Gadsden County. He is a graduate of Sneads High School, Chipola College, and Florida A&M University. He serves the people and families of many small towns in this area like Chattahoochee, Sneads, Quincy, Grand Ridge, Blountstown, Donaldsonville, Marianna, and Bristol.
